QliqHealth is committed to protecting the privacy of your health information. This Notice of Privacy Practices explains how we may use and disclose your protected health information (PHI), your privacy rights, and our responsibilities under applicable law.

When you use QliqHealth, we may collect and maintain information such as your name, contact information, medical history, symptoms, medications, laboratory results, treatment information, care requests, and communications with our clinicians.

To provide, coordinate, or manage your healthcare. For example, we may share relevant information with a specialist, pharmacy, laboratory, or other healthcare professional involved in your care.

To operate QliqHealth, improve our services, support quality and safety, coordinate care, conduct administrative activities, and manage our clinical operations.

We may also use or disclose information when required or permitted by law, including for certain public health, safety, legal, regulatory, and oversight purposes.

When appropriate, we may coordinate your care through secure messaging, phone, video, or in-person care, including referrals to the appropriate specialist or level of care.

AI and Automated Technology

QliqHealth may use artificial intelligence (AI) and automated technology to support patient intake, care requests, clinical workflows, routine communications, care coordination, and administrative tasks.

AI tools may assist with collecting information, organizing requests, or drafting routine communications. AI does not replace our clinicians. When clinical review is required, an experienced clinician reviews the relevant information and makes or confirms clinical decisions.

We use appropriate safeguards for health information processed through technology and applicable service providers.

Your Privacy Rights

You have the right to:

  • Access your health information and request a copy of your medical records.

  • Request corrections to information you believe is inaccurate or incomplete.

  • Request confidential communications, such as asking us to contact you in a particular way or at a particular location.

  • Request restrictions on certain uses or disclosures of your health information.

  • Request an accounting of certain disclosures of your health information.

  • Receive a copy of this Notice in paper or electronic form.

  • Choose someone to act on your behalf when legally authorized.

  • File a privacy complaint without retaliation.

SMS and Electronic Communications

With your consent, QliqHealth may use SMS, email, or other electronic communications for healthcare-related and administrative purposes, such as appointment information, care requests, prescriptions, referrals, laboratory orders, and account notifications.

Do not send detailed or emergency medical information by SMS. Use the secure QliqHealth platform when available.

Care Coordination and Referrals

When your needs require care beyond the services available through QliqHealth, we may help coordinate the appropriate level or specialty of care, including primary care follow-up, urgent or emergency care, or referral to an appropriate specialist.

Substance Use Disorder Records

Certain substance use disorder patient records may receive additional protections under federal law, including 42 CFR Part 2. Where applicable, QliqHealth will handle such records in accordance with applicable federal and state privacy requirements.

Our Use of Service Providers

QliqHealth may use technology, laboratory, pharmacy, communication, payment, hosting, and other service providers to operate our healthcare services. When required by law, we enter into appropriate agreements and require applicable safeguards for protected health information.

Changes to This Notice

QliqHealth may change this Notice from time to time. Any revised Notice will include a new effective date and will be made available on our website.

Privacy at QliqHealth
QliqHealth protects your personal and health information with appropriate privacy and security safeguards. We use your information to provide care, coordinate services, process payments, and communicate with you.

AI & automated tools may support intake, care requests, messaging, and administrative tasks. All care requests and completed intakes are reviewed by a licensed, board-certified, experienced medical provider—including physicians, Nurse Practitioners (NPs), and Physician Assistants (PAs)—before a treatment plan is provided.

When additional care is needed, we may coordinate messaging, phone, video, in-person visits, or referral to the appropriate specialist or level of care.

Your privacy matters. We do not sell your personal or health information for marketing.
